APPLICATION CHECKLIST

 
For Students Re-enrolling in Preschool - Grade 8
1. Completed Application Form
2. Application fee of $75.00 (maximum $115.00 per family) - NONREFUNDABLE (This fee will be applied to next year's tuition or family fees if received by March 20, 2008 for Grades K - 8 or by May 30, 2008 for Preschool classes).
   
Students NEW to Preschool and Kindergarten:
1. Completed Application Form
2. Application fee of $75.00 (maximum $115.00 per family) - NONREFUNDABLE (This fee will be applied to next year's tuition or family fees if received by March 20, 2008 for Grades K - 8 or by May 30, 2008 for Preschool classes).
3. Copy of official Birth Certificate (from the Health Department - not the hospital).
   
Students NEW to Grades 1 - 8:
1. Completed application Form
2. Signed Release of Transcript Form
3. Copy of most recent report card from this school year.
4. Copy of official Birth Certificate (from the Health Department - not the hospital).
5. Copy of most recent competency, achievement, or proficiency test scores
6. (Grade 1 only) - Proof of attendance of Kindergarten
7. Application fee of $75.00 (maximum $115.00 per family) - NONREFUNDABLE (This fee will be applied to next year's tuition or family fees if received by March 20, 2008).
   
For All Applicants:
1. All requirements listed above must be met to begin application process.
2. All applications are dated as they are determined to have all the required items received in the school office.
3. Final approval of applications is done through the principal.
   
Items needed prior to the start of the next school year, as follows:
1. Up-to-date Immunizations. Verification due by the first day of school for Preschool and Kindergarten students.
2. Book and Supply fee (due by August 15, 2008).
3. Up-to-date Emergency Medical Authorization Form, Emergency Medical Card (must be completed yearly; due first day of school), other forms passed out at teacher home visits in August.
